fix(bookkeeping): resolve settlement account in all categorization flows and ship mis-booking audit (#1383)
Completes the #985/#986/#987 caller sweep: categorize-core, v1 batch-categorize, pending-operation edits and the MCP categorize path now resolve the settlement leg from the transaction's cash account instead of inheriting a hardcoded or stale account. Extends the correct_entry preview with currency, tax and dimension line metadata so staged corrections preserve full line fidelity. Adds a read-only audit query and a runbook for reviewing and correcting historical mis-bookings via staged storno with explicit approval; no automated bulk mutation. * fix(assistant): stop cross-user conversation access, bricked threads and lost sessions Hotfix batch (PR1 of the assistant UI makeover, dev_docs/assistant_redesign_plan.md section 7). No visual change; each of these is wrong today regardless of which design lands, and three are unrecoverable per incident. /api/agent/invoke never checked who owns a resumed conversation_id. RLS on agent_conversations/agent_messages is company-scoped, not user-scoped (20260517204000), so a member could post a colleague's conversation id, have their history loaded into the prompt and read it back, while their own turns were appended to that thread. The conversations list route filters on user_id for exactly this reason. Also pins company and intent: resuming a thread from another company would mix ledgers, and resuming under a different intent would swap the tool whitelist under history the model has already seen. A turn persists the assistant message carrying tool_use blocks before the tools run, and their results only after the batch finishes. Dying in between (client disconnect terminating the function, a deploy, a slow tool) left history ending on an unanswered tool_use, which the Messages API rejects on replay: every later turn 400s, and agent_messages is append-only for the BFL trail, so nothing could repair it. History is now patched on read by synthesizing is_error tool_results, leaving the stored trail untouched. check_and_increment_agent_quota is SECURITY DEFINER in public with a caller-chosen p_user_id, so any authenticated user could drain a colleague's minute/day budget and lock them out of every agent endpoint. A plain REVOKE would break the limiter (all three callers use the user's RLS client) and, as it fails open, silently remove the spend cap: the function now refuses to act for anyone but the caller, while service-role connections keep passing an explicit id. The single reject route re-read status and then wrote unguarded, so losing the race with commit's atomic pending -> committing claim stamped `rejected` over an operation that had already posted a verifikat, invisible to the committing-state recovery sweep. Guarded on status like bulk-reject already is; a lost race is now a 409. The sheet's Escape handler listened on window with no defaultPrevented or target check while the sheet is deliberately non-modal, so pressing Esc to dismiss the reject-reason Select inside an approval card, the command palette or any dialog unmounted the sheet and discarded the conversation, the streaming turn and the un-actioned proposal. It now yields to open overlays and to focus outside the sheet. Verified: 9526 unit tests pass, lint clean on touched files, guards pass, and the new pg-real test proves the quota guard against real Postgres (attacker raises 42501, victim counters stay at 0). The four unrelated pg-real failures on this machine reproduce identically with these changes stashed. The caller guard used auth.uid() alone, which is NULL for the `anon` role just as it is for backend roles, so an unauthenticated caller holding the public anon key (it ships in the browser bundle) could still pick any p_user_id and drain that user's quota. The guard now keys on the request role: anon and authenticated may only ever spend their own quota, backend roles keep passing an explicit id. The default PUBLIC execute grant is revoked as a second layer, with execute granted only to authenticated and service_role. Covered by a new pg test for the anon path. The ownership check ran after the onboarding.intake stamp, so a request that was about to be rejected could still write intake_completed_at. It now sits directly after the capability gate, ahead of every side effect and ahead of the company and profile reads, which also makes a rejected request cheaper. The tool-result repair matched ids anywhere in the history, but the API needs results in the message IMMEDIATELY after the tool_use. A result persisted after an intervening turn (two turns racing on one conversation) left a shape that still 400s. The repair is now positional, and orphaned or late-duplicate tool_results are dropped, since an unmatched tool_result is rejected just as an unanswered tool_use is. The Escape guard matched the Radix popper wrapper, which stays mounted when a popper is force-mounted; it now requires data-state="open" so a closed popper cannot block Escape for the rest of the session. Multi-step executors (match_transaction_invoice, credit_invoice) post an irreversible voucher or persist a credit note and then run later fallible steps. fix(pending-ops): recovery sweep for operations stuck in committing (#843) (#1108)
The commit dispatcher claims an op with an atomic pending -> committing CAS; if the process dies after side-effects post but before the terminal committed write (or that write fails, the PR #841 log line), the row sat in status='committing' forever: the expire cron only sweeps 'pending'. Add lib/pending-operations/recover-stuck-committing.ts, invoked from the existing daily expire cron (no new vercel.json entry): - Only rows whose updated_at (the claim timestamp: the CAS bumps it via the update_updated_at_column trigger) is older than 15 minutes, well past the 300s Vercel function ceiling, so in-flight executors are never raced. - Positive evidence that side-effects posted finalizes the row to committed with result_data.recovered=true. Evidence exists only where params identify a target with an unambiguous posted state: categorize_transaction (is_transaction_booked RPC, skipped for allow_duplicate), link_transaction_journal_entry (exact tx+entry link), match_transaction_invoice (invoice_payments pair row). - No evidence: terminal rejected with an explanatory result_data, never back to pending (re-execution could duplicate side-effects that posted without a trace). Reason 'stuck_committing' is distinct from 'expired' so the UI badge never claims these rows. - Every terminal write is CAS-guarded on status='committing'; probe errors skip the row for the next run. - One structured 'pending_op_recovery' warn per row (count by outcome); runbook comment added next to the #841 finalize-failure log line. Tests: unit coverage for the decision logic and cron wiring (401, sweep invoked, failure isolation), plus a pg-real test proving row selection, the trustworthy updated_at anchor, committing -> terminal transitions through the real immutability/input-frozen triggers, and the is_transaction_booked evidence substrate. No user-visible toast or response field can now carry a raw engine or DB message; everything maps through getErrorMessage or the structured-errors registry. - get-error-message: only normalize a code-carrying Error instance into the structured path when the registry knows the code; unknown codes (Node system errors, stray third-party codes, Error-wrapped Postgres SQLSTATEs) fall through to pattern match, Swedish check, Postgres map and the status/context/generic fallbacks instead of returning the raw message. * fix(transactions): allow re-linking a bank tx stranded on a reversed verifikat (#988) A transaction whose journal_entry_id points at a reversed/cancelled entry reads as "utan koppling" in the UI (the transactions page enriches only status='posted' links), yet the re-booking guards treated ANY non-null pointer as "already linked". So a storno'd/corrected transaction could never be linked to another verifikat or re-categorized: the exact symptom in issue #988. Add a shared hasLiveJournalEntryLink() predicate used by every re-booking guard (linkTransactionToJournalEntry, manualLink, categorize-core, and the MCP link stage-check): a pointer at a non-posted entry is treated as re-linkable, and the two optimistic-locked writes now lock on the exact previous pointer (null OR the stale id) instead of always .is(null), so the overwrite goes through race-safely. hasLiveJournalEntryLink fails closed on a read error so a transient blip can't detach a genuinely live link. The source was fixed in #726 (reverseEntry/correctEntry now detach/re-point the tx); this makes the guards self-heal for the pre-#726 backlog and any future best-effort miss. style: remove em/en dashes repo-wide, add CLAUDE.md rule against them (#890)
Em dashes (—) and en dashes (–) had spread across comments, docs, tests, and a few UI strings, reading as AI-generated boilerplate rather than house style. Replaced each with punctuation matching its context: colon for explanatory clauses, comma for asides, plain hyphen for numeric/legal ranges (e.g. "21-23§"), "to"/"till" for date ranges, parentheses for paired-dash asides. messages/en.json and messages/sv.json were fixed by hand together to keep sv/en in sync. Left untouched where the dash is the functional subject rather than decorative punctuation: date-range-parser.ts's separator regex, charset-repair.ts's CP1252 byte-mapping table (and its test), the SIE encoding mojibake docs, generic-csv.ts's minus-sign normalizer, the agent system-prompt files that already instruct against em dashes, and a golden iXBRL test fixture compared byte-for-byte. Also fixes two bugs surfaced along the way: an off-by-one in ApiKeysPanel's scope-label split (a leftover from an earlier partial pass), and a charset-repair test that had lost the literal en-dash it exists to verify. Regenerated the agent atom seed migration (skills:generate) since 27 SKILL.md files changed. fix(reports): count reversed entries in bank/supplier/AR reconciliation (#734)
A reversed (storno-corrected) journal entry must be summed together with
its storno + correction, exactly as the trial balance and balance sheet
already do (.in('status', ['posted','reversed'])). The reconciliation
paths used posted-only and manufactured phantom differences.
- bank-reconciliation getReconciliationStatus: gl_1930_balance now counts
posted+reversed (so it equals the balansräkning for the account). Dropped
the correction_adjustment subtraction and the reversed-linked-tx drop — a
corrected/amount-corrected bank receipt now reconciles. The prior model
broke once correctEntry began re-pointing the bank transaction to the live
correction (the two changes were mutually inconsistent and produced a
difference equal to the corrected amount).
- supplier-reconciliation (2440) and ar-reconciliation (1510/1513): same
posted -> posted+reversed fix. Removes the false "Ej avstämd" gap a fully
paid, fully corrected company shows — the books net to 0 over posted+reversed
while a posted-only query double-counted the payment legs.
Also in this change:
- MCP gnubok_get_reconciliation_status: add account_number param (was hardcoded
to 1930; the lib already supported per-account reconciliation).
- counterparty-templates normalizeCounterpartyName: strip trailing month /
personal-initials tokens so "ngrok JW" / "Ngrok Mars" learn as one merchant.
- pending-operation reject 409 (route + MCP tool): clarify a resolved op was
approved explicitly (no auto-commit path exists) instead of a bare
"already committed".
- create_voucher: normalize description with String().trim() for consistency
with line_description.
- migration 20260625120000: backfill stripped diacritics on ~830 companies'
seeded chart-of-accounts names (Foretagskonto -> Företagskonto, etc.); the
seed function was fixed for new companies in 20260516130000 but never
backfilled. UPDATE-only and idempotent. Already applied to production.
Tests: 899 passing across the touched suites; lint + typecheck clean.

The categorize flow always derived VAT as rate × gross/(1+rate) from the
transaction amount, with no way to use the underlag's actual moms. On e.g.
a restaurant receipt with dricks (no VAT on the tip), the agent could see
the document's correct VAT but the staged booking recomputed the wrong
rate-based amount on every attempt.
- buildMappingResultFromCategory: optional vatAmountOverride replaces the
rate-derived VAT line ("Ingående/Utgående moms (enligt underlag)"; 0 =
no VAT line). Rejects negatives, amounts above the 25%-extraction bound,
and combination with reverse_charge / VAT-less treatments / private.
- gnubok_categorize_transaction: new vat_amount input, threaded into the
staged preview and persisted in the operation params.
- commitCategorizeTransaction: reads params.vat_amount so the approved
posting matches the staged preview exactly.
- PATCH /api/pending-operations/[id]: accepts vat_amount (null clears);
preserves a staged override across category edits while the treatment
still carries rate-based VAT, drops it when it no longer does.

booking and the correct income account (3004), so 0 had no use case and
only created a silent momsdeklaration misclassification path. Schema
declares exclusiveMinimum: 0.

* fix(sie-import): accept tab as field separator (Bollbok exports) The SIE 4 spec allows either space or tab between fields, but splitSIELine() only treated space (0x20) as a separator. Bollbok exports tab-separated lines for every record except #RAR, which silently swallowed all #IB / #UB / #KONTO / #KTYP / #VER / #TRANS records — imports appeared empty even though the file was well-formed. Also adds a parser-side diagnostic that emits a warning when raw #IB or #VER lines are present in the input but parsing produced none. The previous silent failure is how this bug stayed hidden; the warning gives the import preview something visible to surface next time. Verified against two real reproducer files (Sean / Erik Hellqvist): erik h 2025.SE (UTF-8): 166 accounts, 66 IB, 4 UB, 11 RES, 95 vouchers, 198 TRANS. erik h 2026.SE (CP437): 166 accounts, 66 IB, 4 UB, 0 vouchers. Booking to an account not in the active chart previously threw a
generic 400 "Account(s) not found: 5010" and the user had to leave
the form to enable the account via /bookkeeping > BAS-katalog.
- New AccountsNotInChartError thrown from resolveAccountIds in the
engine (and the parallel resolver in core/storno-service). The
query also now filters on is_active=true, so deactivated accounts
are treated the same as never-added ones.
- API routes that call the engine (journal-entries, reverse, correct,
transactions/book + match-invoice + match-supplier-invoice +
uncategorize, invoices/mark-paid, supplier-invoices + mark-paid +
credit, salary/runs/correct, import/opening-balance/execute,
pending-operations/commit) catch the typed error and return a
structured 400: { error: { code: ACCOUNTS_NOT_IN_CHART,
account_numbers, message } }.
- /api/bookkeeping/accounts/activate now also reactivates rows that
already exist but are is_active=false, not only INSERTs. Returns
{ activated, reactivated, skipped, unknown }.
- New GET /api/bookkeeping/accounts/bas-lookup?numbers=... resolves
BAS names client-side so the dialog can show "5010 · Lokalhyra"
without bundling the full 1,276-account catalog.
- ActivateAccountsDialog lists the missing accounts (BAS names + any
unknown non-BAS numbers) and confirms with a single action.
- useSubmitWithAccountActivation wraps an async submit: on
ACCOUNTS_NOT_IN_CHART it opens the dialog, activates on confirm,
then retries the original submit so the user never re-enters data.
- AccountCombobox accepts any 4-digit numeric value, not just items
from the active chart — the activation dialog handles the rest.
- JournalEntryForm wired to the hook + dialog. Other submit surfaces
now surface a clear Swedish message ("Följande konton behöver
aktiveras: …") via getErrorMessage; wiring the dialog into those
is an additive follow-up.

Seven fixes based on Greptile + Swedish compliance review on #308.
- ActivateAccountsDialog: disable the confirm button when any
entered number isn't a valid BAS account. Previously activation
would succeed for the knowns and the retry would immediately
fail again on the unknowns, giving a confusing double-toast UX.
- pending-operations/commit: revert commitSendInvoice and
commitMarkInvoiceSent to swallow AccountsNotInChartError
silently. The prior PR upgrade made these blocking, which
regressed invoice delivery for users whose AR accounts are
inactive — and since the activation dialog isn't wired into
those flows yet, there's no one-click recovery. The silent
catches now append an InvoiceJournalEntrySkipped event to
processing_history so the missing verifikation is actionable
in audit trails rather than silently understating the
momsdeklaration (revenue / utgående moms unposted).
- engine.reverseEntry: resolve account IDs with includeInactive=true
so storno of an already-committed entry goes through even when
the user has since deactivated one of its accounts. Blocking
the reversal would leave the original entry uncorrected in
violation of BFL 5 kap 5§ (rättelse must be documented). The
default (includeInactive=false) still applies to createDraftEntry
so new bookings to inactive accounts continue to trigger the
activation dialog.
- supplier-invoices POST + credit: roll back the just-inserted
supplier_invoices row (items cascade-delete) on any JE failure,
not only AccountsNotInChartError. An orphan supplier_invoices
row without a registration / credit JE leaves leverantörsskuld
(2440) and ingående moms (2641) unposted — a silent
understatement / overstatement in the momsdeklaration (ML
2023:200 / BFL 5 kap). The catch now returns a clear Swedish
error message for non-activation failures (typically period
lock or DB error) instead of silently logging.
Test mocks for chart_of_accounts updated for the new query chain
(eq.in.eq instead of eq.eq.in after the is_active conditional).
