fix(security): audit remediation 2026-09-01: api_keys identity, viewer gates, OAuth binding, XSS, MFA gate (#2155)
* fix(security): bind api_keys to the caller, lock hash-as-bearer RPCs and provider token tables Security audit 2026-09-01, critical items. - api_keys INSERT requires user_id = auth.uid() again (an admin could forge a key for any co-member and act as them in every company they belong to); SELECT is own-keys-or-admin; a BEFORE trigger freezes the identity and credential columns against user-session UPDATEs. - rotate_mcp_refresh_token and validate_and_increment_api_key become service_role only: they match rows by a presented SHA-256, so a hash readable by co-members was a bearer credential. - validate_and_increment_api_key fails closed when the key's user is no longer a member of the key's company. - provider_consent_tokens and provider_otc: the DELETE policies collapsed to "caller has any team row" (correlated subquery on a non-existent team_members.company_id). All member policies dropped; service_role only, matching every existing code path. fix(observability): clear the dead session on the first bounce, stop filing client disconnects as exceptions, scope the Hem probe (#2114)
Three small defects found underneath error clusters that are themselves benign. bounceToAuth returned a bare NextResponse.redirect and never copied the cookies off supabaseResponse, so the Set-Cookie headers that clear a dead session were thrown away on the first bounce and the browser replayed the dead refresh token once more on /login. That doubled both the GoTrue 400s and the log volume. The AuthApiError itself is left alone: it is correct session-expiry handling that auth-js logs from inside node_modules, and getUser() returns it as a value. "The destination stream closed early." is a client disconnecting mid-stream, produced inside React's Flight server. Next's own isAbortError filter does not recognise React's cancel error, so instrumentation.ts reported it to PostHog Error Tracking as a real exception against real users' session replays and paid an awaited flush on an otherwise-healthy request. A narrow predicate now early-returns before PostHog is touched. This cannot remove the line from Vercel's runtime-error table, which is fed by Next's stderr. other-account-hint.ts issued an unfiltered journal_entries probe on the blocking Hem render path, inside a render Promise.all: roughly 1 in 20 Hem loads waited an extra 2.5 s for an advisory nudge. An unfiltered probe on a multi-tenant table is also a correctness smell. It is now company-scoped and off the blocking path. fix(auth): scope the home-domain ok cookie to the signed-in user (#2083)
The gnubok-home-ok cache cookie stored only the host, so any account that signed in within the 15-minute TTL window inherited the previous user's "this is home" verdict in the same browser and skipped the brand-host bounce entirely: a user with no ties to a white-label brand could land inside the branded shell instead of being redirected to the canonical domain (amnas account-switch repro, two logins 9 s apart). The cookie value is now userId~host and the middleware only skips the affinity check when both match the current session. Old host-only cookies never match, so the check re-runs and the format self-migrates; no sign-out hook is needed because a foreign verdict misses by construction. A signup-allowlisted partner owner (brand_signup_allowlist) could sign up on an invite-only brand domain but was then exiled to the canonical domain by the middleware home-domain affinity rule before any team membership existed: the layout-level exemption in resolveBrandDomainBounce was unreachable because the middleware redirect runs first. Rule 2 now checks the allowlist (fail-closed) and lets those sessions stay, caching the verdict like other home verdicts. Found via the Ziffr owner lockout. fix(errors): keep the SQLSTATE when wrapping database errors (#2027)
isTransientFailure() checks the driver's error code first, and 57014
(statement timeout) is already in its transient set. But the wrapping idiom
across the codebase was `throw new Error(\`Database error: ${err.message}\`)`,
which keeps the prose and drops the code. A retryable timeout therefore
arrived anonymous and resolved to UNKNOWN_ERROR: "Något gick fel. Försök
igen." An agent cannot dispatch on that, so it retried.
On production over 60 days, with the two bot integrations excluded: 1024 real
agent failures, 645 of them UNKNOWN_ERROR across 60 actors and 57 companies.
82 retry streaks of three or more identical failures, 462 wasted repeat calls,
53.1% of all agent error calls sitting inside a streak.
The worst offender traces to one line in core. gnubok_query_journal failed 164
times at a p50 of 8110ms while every other failing tool sat between 1 and
315ms, and its path is fetchEntryLines -> fetchAllRows, where
lib/supabase/fetch-all.ts threw `new Error(error.message)`. That is the
highest-traffic strip point in the repo: 31 callers, every paginated read.
query_journal already had a correct TRANSIENT_ERROR branch offering "retry, or
narrow with date_from/date_to" which could never fire, because by the time it
looked, the code was gone.
fetch-all keeps the driver message verbatim: callers match on the existing
text, and this adds the code rather than rewording anything.
Attaching the code is safe. extractCode() only accepts /^[A-Z_]+$/ and every
SQLSTATE contains digits, so it cannot be mistaken for one of our own stable
codes. There is a test for that, and one asserting the old bare-Error shape
still resolves to UNKNOWN_ERROR so the fix cannot silently regress.
Also stops rendering the literal "undefined" when a driver-level failure
carries no message, which is the string that made these unsearchable.

perf(auth): skip the MFA factor lookup once the session is at AAL2 (#1933)
The enforced-MFA branch of the proxy called supabase.auth.mfa.listFactors() on every page, RSC and prefetch request for every MFA-verified user with a company. auth-js implements listFactors() as a getUser() network round trip, so hosted page requests paid two Supabase Auth calls in sequence. At AAL2 a verified factor exists by construction (the session got there by verifying a challenge on one), so the lookup only runs on the aal1/aal1 path (users mid-enrolment), where it still gates exactly as before. The one case deferred is a user who unenrols their last factor mid-session: the JWT keeps aal2 until the next refresh, so the enrolment bounce lands on the refresh instead of the next click. Tests: aal1/aal1 still asks for the factor list and bounces to /mfa/enroll; at aal2 listFactors is never called (even with a factor list that would read as empty) on page, RSC and prefetch requests; the step-up bounce and the no-company skip are unchanged. The proxy in front of every page, RSC, prefetch and /api request makes several sequential network calls (getUser, session state, the resolve_active_company RPC, MFA factor lookups) and nothing measured them, while the route wrapper has logged authMs/companyMs/handlerMs per API call for months. This is the first PR of the responsiveness plan (customer report: "it takes time before all fields load when clicking around"): the baseline every later change is measured against. - lib/supabase/proxy-timing.ts: pure helpers (request classification from the app-router headers, route template that collapses ids and tokens, Server-Timing formatting, a timed() accumulator). - lib/supabase/middleware.ts: updateSession wraps updateSessionInner, times each phase, sets Server-Timing on page/RSC/prefetch responses and X-Proxy-Timing on /api responses (withRouteContext owns Server-Timing there), and emits one "proxy completed" log line per request. - scripts/perf/log-percentiles.ts: p50/p90/p99 per group over `vercel logs --json` output, for both "op completed" and "proxy completed"; scripts/perf/README.md documents the protocol and targets. `autoRefreshToken` defaults to true in supabase-js, and off-browser
@supabase/auth-js starts the refresh ticker unconditionally:

That is a setInterval firing every 30 s. It calls unref(), so the process
still exits, tests pass, and Vercel never notices because the process is
torn down long before the tickers accumulate. But unref() does not make a
timer collectable: it stays registered in the event loop and remains a GC
root for its callback, which closes over the GoTrueClient, the
SupabaseClient, and the whole request scope around it.
A long-running self-hosted instance therefore leaks one timer plus one
entire request graph (socket, IncomingMessage, ServerResponse, headers,
route context: ~100 kB) per client constructed. One died of "JavaScript
heap out of memory" after 42 h, the last 24 of them completely idle. The
heap snapshot showed 445 retained request graphs and ~1050 Timeouts in
the 30 000 ms bucket, retained via `autoRefreshTicker`, and the rate
matched the traffic exactly: the Docker healthcheck polls /api/health
every 30 s and the webhook dispatch cron runs every minute, so
3 clients/min x 148 min = 444.
- new lib/supabase/service-client.ts: createServiceRoleClient() applies
SERVER_AUTH_OPTIONS, spread LAST so a caller passing its own auth block
cannot re-enable the ticker
- 22 call sites migrated; only booking-templates/sync/cron had ever
passed the options itself
- guard 9 in no-new-antipatterns.mjs fails CI on any new value import of
supabase-js's createClient outside the wrapper; type-only imports are
fine. Verified to fail on a deliberate regression and pass once fixed
- browser clients untouched: a signed-in tab genuinely needs the refresh,
and lib/supabase/client.ts is built on createBrowserClient anyway

Namespace value imports are treated as leaky outright rather than tracking
member access, which keeps the check a regex over source text with no new
dependency.
Review also suggested excluding *.test.tsx alongside *.test.ts. Skipped: the
repo has no .test.tsx files, and all four sibling checks in this file use
`.test.ts`. Diverging in one of them would read as an accident; if such files
appear, all four should change together.

The middleware allowlist never included the agent-discovery surfaces, so every anonymous request to /llms.txt, /llms-full.txt, or /docs/* was 307-bounced to /login on hosted. The llms.txt convention exists for logged-out crawlers and IDE agents, and /docs is the public API documentation that the OpenAPI spec and the installable accounted-api skill link to. openapi.json and /.well-known/* only escaped because the proxy matcher skips .json and .well-known paths. Signed-in users fall through to the same content with no redirect. The policy compares the company segment as text rather than casting to uuid the way sie-files does: this bucket holds keys whose second segment is not a uuid (MCP audit packages), and Postgres does not guarantee the bucket prefix qual runs before the cast, so a planner reordering would raise 22P02 and fail the whole query instead of filtering the row out. deleteDocument now removes both candidate keys. Removing only the stored pointer would leave a readable orphan copy of a document the user asked to erase. The backfill script is included but has never been run. It defaults to dry run, refuses .env.local by name, and verifies each copy is readable and SHA-256 identical before repointing the row. Driving from the entries side instead turns that into two indexed round trips. Converted sites keep their existing shape: the helper reattaches the parent entry under the same key the embed produced. Several conversions also remove a latent silent truncation where an unpaginated query was capped at PostgREST's 1000 row ceiling. Two deliberate exceptions. The free text ilike legs of the MCP display query stay on the embed, because each is capped at legLimit and that cap drives the truncation contract the tool reports, while the helper is unbounded. The accounts route moves to the existing get_account_usage_counts RPC instead, since its embed was a head count and the helper returns rows. commitEntry's write path is untouched: the change there is confined to the read query of the pre-commit dimension rule check. The tests use an in-memory PostgREST that actually evaluates the filters, because the repo's pass-through mock cannot catch this class of bug: the bug is that the filter is never sent. They walk to exhaustion with a hard iteration cap, so an unterminated walk fails instead of hanging. An agent or integrator saw success for a write that never happened. dry_run is folded into the hash only when true, not as an unconditional boolean. Including it as false would change the hash of every ordinary write, and with a 24h idempotency TTL any key in flight across the deploy would fail the request_hash comparison and 409 on a legitimate retry. Both hash call sites now go through one shared helper so they cannot drift into a permanent cache miss, and dry run responses are no longer stored at all. With --no-package-lock npm discarded the lockfile and re-resolved the whole tree from package.json, floating @hookform/resolvers to 5.4.3, whose valibot ^1 peer conflicts with the pinned valibot 0.39.0. Installing into the parent of the checkout resolves only that one package, so an unrelated peer conflict can never take the gate down again. Node still finds it because ESM bare specifiers walk up parent node_modules; NODE_PATH would not have worked, as it is CommonJS only. --legacy-peer-deps was rejected because it masks future genuine peer conflicts and still reifies the full tree. The same step's SDK version is aligned from 0.31.0 back to the 0.29.1 that package.json and check:guards enforce after the streaming outage. That drift went unnoticed because the pin guard only inspects package.json and the lockfile, never workflow files. Self hosted deployments therefore never sent recurring invoices, never dispatched webhooks and never cleaned up idempotency keys. tax-deadlines also ran once a year on 2 January instead of daily, and documents/verify weekly instead of daily. Extension crons are included rather than excluded. The Dockerfile copies the whole tree before building, so every extension cron route is compiled into the image regardless of the enabled preset, and each returns 200 when its extension is unconfigured, so curl -sf logs no failure. Two such entries were already present in the crontab for extensions absent from the preset, which settles the intent. documents/verify is treated as drift rather than a self hosted concession: the weekly cadence was present in the hosted crontab too, and the run is capped at 200 documents walking a nulls-first queue, so weekly drains the integrity queue seven times slower on a check that exists for BFL retention. webhooks/dispatch keeps its per minute cadence, adding 1,440 requests a day on self hosted. A gentler tick would silently stretch the first retry, since the retry ladder opens at 60 seconds. SCHEDULE_OVERRIDES is the one line place to change that. A parity test asserts the path sets match minus a documented exclusion list, and ratchets three cron routes that are currently scheduled nowhere so they are named rather than silently rotting. The two most recent bug fixes on this repo were both discovered by customer email. This adds the sink, not a vendor. No dependency is taken: the interface has a no-op default and a registration point, so behaviour is unchanged until an adapter is registered. Releases are tagged from the build id already inlined by next.config.ts. Redaction moved out of lib/logger.ts into a leaf module that both the logger and the sink import, so there is one denylist and no path from application data to a third party can skip the personnummer regex, including direct sink calls that bypass the logger. That matters here because these logs carry personnummer and financial data. verifyCronSecret now reports its own 401s, which covers all 16 jobs without touching a route file and catches the case where CRON_SECRET is rotated without updating the scheduler and every job silently 401s forever. The threshold is one failure rather than the backup alert's three: suppressing the first occurrence is precisely how an outage stays invisible. The seven misleading comments are corrected to describe what the code actually does, including the two cases that still are not covered: the client side one, since the sink is server side, and a warn level call that is not forwarded. style: remove em/en dashes repo-wide, add CLAUDE.md rule against them (#890)
Em dashes (—) and en dashes (–) had spread across comments, docs, tests, and a few UI strings, reading as AI-generated boilerplate rather than house style. Replaced each with punctuation matching its context: colon for explanatory clauses, comma for asides, plain hyphen for numeric/legal ranges (e.g. "21-23§"), "to"/"till" for date ranges, parentheses for paired-dash asides. messages/en.json and messages/sv.json were fixed by hand together to keep sv/en in sync. Left untouched where the dash is the functional subject rather than decorative punctuation: date-range-parser.ts's separator regex, charset-repair.ts's CP1252 byte-mapping table (and its test), the SIE encoding mojibake docs, generic-csv.ts's minus-sign normalizer, the agent system-prompt files that already instruct against em dashes, and a golden iXBRL test fixture compared byte-for-byte. Also fixes two bugs surfaced along the way: an off-by-one in ApiKeysPanel's scope-label split (a leftover from an earlier partial pass), and a charset-repair test that had lost the literal en-dash it exists to verify. Regenerated the agent atom seed migration (skills:generate) since 27 SKILL.md files changed. * fix(reports): stabilize fetchAllRows paging to stop doubled/dropped balances (#790, #791) PostgREST `.range()` paging is only correct when the underlying query has a stable TOTAL order. Several aggregating report queries (general ledger, trial balance, grundbok, supplier/AR ledgers, etc.) paginated without `.order()`, so on datasets larger than one 1000-row page Postgres could return rows in a different order between requests — silently DUPLICATING or SKIPPING rows on a page boundary and doubling or dropping financial totals. - fetch-all.ts: document the ordering invariant and add an optional `dedupeBy` defense-in-depth that drops cross-page duplicates and warns when it fires (surfaces a missing `.order()` in logs instead of corrupting money). - Add a stable `.order()` (line PK or account_number) to every paginated query in lib/reports/ and the account-balances route; pass `dedupeBy` on the money-aggregating line queries. - Add fetch-all unit tests and update report test fixtures to carry row ids.
