* chore: repo-wide bloat sweep, remove dead code and fold duplicate helpers Remove 33 dead files, ~270 unreferenced exports/types, 13 dead i18n namespaces and 4 unused dependencies; fold byte-identical helper copies into one canonical home each (lib/utils chunk/sleep/utcDateStamp, lib/dates/iso, lib/invariants/uuid, lib/xml/escape, lib/reports/sru/format, lib/pdf/number-text, lib/browser/panel-request, lib/api/v1/body + v1ValidationError rolled out to ~55 v1 routes, booking-template schemas). No behaviour change: v1 bodies and status codes, MCP tool schemas, DB writes and money math are untouched. Naive ore rounding was deliberately not swapped for roundOre; see DECISIONS.md 2026-09-02 for the full list of things left alone on purpose. tsc, lint, 19588 unit tests and check:guards green; antipattern baseline ratcheted (naive-ore-round 622 -> 620, hand-rolled-invariant 115 -> 113). fix(security): audit remediation 2026-09-01: api_keys identity, viewer gates, OAuth binding, XSS, MFA gate (#2155)
* fix(security): bind api_keys to the caller, lock hash-as-bearer RPCs and provider token tables Security audit 2026-09-01, critical items. - api_keys INSERT requires user_id = auth.uid() again (an admin could forge a key for any co-member and act as them in every company they belong to); SELECT is own-keys-or-admin; a BEFORE trigger freezes the identity and credential columns against user-session UPDATEs. - rotate_mcp_refresh_token and validate_and_increment_api_key become service_role only: they match rows by a presented SHA-256, so a hash readable by co-members was a bearer credential. - validate_and_increment_api_key fails closed when the key's user is no longer a member of the key's company. - provider_consent_tokens and provider_otc: the DELETE policies collapsed to "caller has any team row" (correlated subquery on a non-existent team_members.company_id). All member policies dropped; service_role only, matching every existing code path. * fix(bank): never pre-check or mirror another company's accounts in the EB callback At one-session banks (SEB) the PSU's single consent can cover accounts a sibling company books. The OAuth callback stored whatever the session returned into the active company: all pre-enabled, mirrored into its cash_accounts, ledgers allocated from its chart: one 'Spara val' away from booking another aktiebolag's transactions (user report F1, 2026-09-01). The deliberate reuse path (findReusableSessions) already guards claimed IBANs; the callback now runs the same check via fetchCrossCompanyAccountContext: - accounts claimed by another of the user's companies are stored disabled + flagged (claimed_by_company_*), skipped by the cash_accounts mirror, and the picker names the claiming company - a 'Synkas ej' deselection made on any other connection row is carried onto fresh rows (the recurring came-back-pre-checked complaint, C2) - lookup failure fails closed: new accounts stored deselected - accounts the row itself already carried keep their own state, so a renewal can never switch a working feed off Co-Authored-By: Claude Fable 5 <noreply@anthropic.com> Claude-Session: https://claude.ai/code/session_0197wmwP6zNaYvsGfbZuQHGA * fix(bank): close the skeptic-found holes in the cross-company claim guard Consolidated fixes from the three-skeptic review of PR #2116 (all three refuted the first cut): - Active-company standing state (enabled cash_accounts + enabled accounts on its live-ish connection rows) now outranks sibling claims company-wide, not row-wide: a bank-list renewal arrives on a FRESH row with no priors, and the old row-local check would have let a sibling claim switch a working feed off while supersede demoted its cash row. - pending_selection rows no longer claim accounts or feed deselection memory: their flags are unconfirmed callback output (including this guard's own fail-closed writes), so an abandoned picker or a transient lookup error can no longer poison later connects. - Guard-disabled accounts are never mirrored from the callback: upsertFromPsd2 with enabled:false for a new-to-row account could promote the seeded primary 1930 manual row and flip it to disabled under a foreign identity. - The selection save skips ledger allocation and the cash_accounts mirror for disabled never-mirrored accounts, so 'no cash row, no 19xx slot burned' holds past the mandatory Spara val, and strips the claimed_by_*/deselected flags when the user deliberately enables an account. - Deselection carry is no longer silent: deselected_elsewhere flag + picker note 'Tidigare bortvald'. - Claim lookups paginate via fetchAllRows: the bare select's silent 1000-row PostgREST cap failed open for exactly the multi-company consultants the guard exists for. feat(connect): Enable Banking client routes through the hosted proxy in connector mode (PR6b-1) (#2094)
* feat(connect): route the Enable Banking client through the hosted proxy in connector mode PR6b-1 of the instance-side client wiring. Until now bankConnectorMode() had no consumer but the status label; this makes a self-host with a connector key and no own EB credentials actually reach Enable Banking through the hosted bank proxy. - api-client authenticatedFetch: in connector mode swap the base URL to the proxy and send the connector key as a Bearer token. The EB JWT signer (getAuthorizationHeader) is never called: the instance holds no private key. On hosted and on own-credentials self-hosts the direct path is byte-identical. - startAuthorization forwards X-Connector-Company so the proxy can meter the per-company connection quota; index.ts passes companyId at both connect sites. - createSession forwards the signed connector_state so the proxy binds the /sessions exchange to the pending ledger row (single-use, race-safe). - callback route reads connector_state from the query (echoed by the hosted callback) and threads it through finalizeConnection. Tests: connector-mode base/auth/company-header/connector_state assertions in api-client, direct-path and own-credentials byte-identity, and the callback threading both connector and direct paths. Issue #1716: a user stuck in Handelsbankens fullmakt step got a raw provider token back (server_error, invalid_state) and support had nothing to look at afterwards: the failed pending row is deleted by design, the callback only logged to console (short retention), and event_log recorded successes only. Diagnosis of the reported case: the failures were on the bank's side (the corporate fullmakt requirement); both of the reporter's companies connected successfully on 2026-08-12 with no code change on our side in between, and the connections have been active and syncing since. Changes: - lib/errors/get-error-message.ts: getBankConnectionErrorMessage() maps PSD2 callback outcomes (access_denied, server_error, temporarily_unavailable, session expiry, plus the internal invalid_state, missing_parameters and invalid_code_format tokens) to Swedish user messages, appending the raw provider description so the underlying error is still surfaced. - callback route: every bank_error redirect and the stored error_message now carry the mapped Swedish text; bank_error_code, bank_name and psu_type still flow so the settings page keeps its targeted guidance (Handelsbanken fullmakt steps included). - New audit events bank_connection.consent_denied and bank_connection.finalize_failed are emitted on the two failure paths and persisted to event_log, so support can answer which attempt failed, with which provider error, on whose side, even after the row is gone. Root cause (issue #1709 residual corner): on an in-place reconnect, the callback carries each account's external_id dedup scope by matching prior accounts by IBAN or uid. A no-IBAN account whose ASPSP minted a new uid matched neither, so it got a fresh scope: every historical external_id regenerated, Layer-1 dedup missed the re-import, and the whole history came back as new unbooked rows. The cash_accounts mirror then could not find the old row either and allocated an overflow 19xx slot plus a NEW row, which also blocked the content-dedup bridge's account guard. Fix: pair such accounts by elimination, only when unambiguous (per currency, exactly one unclaimed prior and exactly one fresh-scope new account, neither with an IBAN): carry the prior scope and enabled flag, and reuse the connection's own old cash_accounts row via the existing explicit reuse_cash_account_id promote path in upsertFromPsd2, so the row id, ledger, and transaction links survive the uid change. Any ambiguity keeps the previous fresh-scope behavior. Also count account-incompatible same-feed orphaned ids in the scope-drift shadow (log-only) so fleet validation can see this incident class. SEB mints new account uids on re-auth, so no uid matched, the IBAN hit on the old row was rejected by its own ledger being excluded, and a fresh 195x slot was allocated (and created in the chart) on every renewal. Only ledgers still claimed by a uid present in the new session are excluded now; the stale row is promoted via the IBAN match as intended. Stale ledgers stay safe from the allocator, which already skips every cash_accounts ledger. 2. Deselected accounts came back pre-checked. accounts_data was rebuilt with enabled:true unconditionally, so a private card set to "Synkas ej" was re-enabled and the mirror flipped cash_accounts.enabled back. The prior flag is now carried over by IBAN, then uid; only genuinely new accounts default to enabled. A renewal performed via the bank list ("Anslut ny bank") created a second
bank_connections row and left the old one parked in 'expired' forever: an
eternal "Åtgärd krävs" card, a red status chip, transactions stranded on the
dead row (so the picker's gap-fill probe read the renewal as a first
connect), and re-imported history for no-IBAN accounts whose provider uids
change on re-authorization.
- New migration: additive superseded_by uuid (FK, ON DELETE SET NULL) +
superseded_at + partial index on bank_connections. Status 'revoked' is
reused for superseded rows (no CHECK change); superseded_by disambiguates
a supersede from a user disconnect. File only: not applied anywhere yet.
- New lib/supersede.ts: after the OAuth callback finalizes, park same-bank
siblings matched by IBAN overlap (an ACTIVE sibling without overlap is
never touched; no-IBAN fallback only for dead siblings when neither side
has IBANs), revoke their EB session only when countLiveSiblings says
nobody shares it, re-point their transactions in id batches, demote
leftover cash_accounts claims (the mirror then promotes them by IBAN),
carry last_synced_at + initial_sync_* onto the survivor, and emit the new
bank_connection.superseded audit event.
- /connect fresh path: 409 { code: 'EXISTING_CONNECTION',
existing_connection_id } when a non-revoked same-bank row exists, unless
the body carries force_new: true (escape hatch for a second login at the
same bank). Runs after the zombie sweep; reconnect-in-place unaffected.
- Dedup scope stability: StoredAccount.dedup_scope pins the external_id
account scope at first ingest (normalized IBAN, else the uid of that
moment), is carried across in-place reconnects and supersedes by IBAN
match, and sync.ts uses dedup_scope ?? IBAN ?? uid (stamping legacy rows
lazily). The external_id FORMAT is untouched.
- AccountPickerDialog gap-fill probe also includes superseded connection
ids so the renewal default never races the transaction re-point.
- Sync toast (BankSyncNowButton) now also reports skipped duplicates
(sv+en strings) so a correctly deduped renewal does not look broken.
Tests: supersede unit tests, /connect 409 + force_new, callback supersede
wiring + dedup-scope carry, sync external_id stability across uid changes.

the revoke entirely (logged) when the park update fails, so a failed park
can no longer leave a live-looking row with a dead session.
- callback keeps a survivor account's explicit dedup_scope instead of
letting a carried sibling scope clobber it; carried scopes only apply
when the survivor's scope was derived (IBAN/uid fallback).
- sync-now toast joins its two sentences with '. ' so the imported and
skipped-duplicates messages no longer run together.

Prod says this is not one customer: every SEB customer holding connections in more than one company has had an earlier company stop syncing at the moment the next was authorized, most of them while the consent was still formally valid for weeks. The same measurement over other banks is far quieter, so the one-active-session-per-PSU limit is real and ASPSP-side. Enable Banking already supports the shape we want. POST /auth carries no account restriction, so a session covers every account the user ticked at the bank, and GET /accounts/{uid}/transactions takes no session id, so a second company can sync its own accounts from an existing session. bank_connections has no unique constraint on session_id, so this needs no migration. Adds lib/session-sharing.ts plus GET /reusable-sessions and POST /attach. When a live session in another of the user's companies still exposes accounts no company syncs, the settings panel offers to reuse it: the new row shares session_id and consent_expires, carries only the unclaimed accounts, and lands in pending_selection so the existing IBAN-aware account picker does the ledger mapping. Only the consent is shared; accounts, cash_accounts and transactions stay strictly per-company. Sharing a session changes three lifecycle paths, all handled here: - Disconnect and reconnect now refcount before revoking. A blind revoke would take down a sibling company's feed, which is the exact failure this removes. The count runs on a service-role client because RLS hides a sibling in a company the user has since left, and it fails closed: an uncertain count is treated as shared, since a lingering consent lapses on its own in 90 days while a wrongly revoked one kills a working feed. - A renewed consent fans out to every company sharing the old session, and re-points their account uids by IBAN. Several ASPSPs reissue uids on re-authorization, so carrying the session id alone would have left siblings calling retired uids and re-broken them every quarter. This is also why the superseded session_id is no longer nulled at /connect: the callback needs it. - The nightly probe runs once per distinct session and applies the verdict to every row holding it, and expiry mails are keyed per (user, session), so one dead consent is one probe and one mail rather than four of each. Only enabled cash_accounts rows count as claiming an IBAN. The callback mirrors every account in a consent, deselected ones included, so counting any row as a claim would leave nothing offerable once the first company connects. An account handed to a company also stops being offered while that company's picker is still open, closing the window where two companies could book the same physical account. The visual panel is deliberately not built here: it needs a /frontend-design pass against the locked concept conventions plus sv/en strings, and the warning above already puts the alarm where the user looks. CodeRabbit, statement-reconciliation: resolveEntityType checked neither query's error, so a genuine DB failure (RLS, permissions, connectivity) returned null indistinguishably from "no entity type set", fell into the unsupported-form branch and reported isReconciled: true. That is the same silent-false-reconciled bug the cycle-1 refactor closed, one level down. The companies error now throws; a missing company_settings ROW stays tolerated, because .single() errors on zero rows and many companies have none. Mirrors the pattern the INK2 and NE engines already use. Still open by Emil's explicit choice: the consent-expiry cooldown is stamped only on the connection it was handed, so it leaks one duplicate mail per sibling company on the shared session. * fix(enable-banking): keep bank account mappings across reconnects and surface dead sessions A PSD2 reconnect silently moved the user's ledger mapping. Account identity came from the provider's account uid, which does not survive a re-authorization at every ASPSP, and a fresh connect to an already-connected bank mints a new bank_connections row regardless. Both paths looked like "an account we have never seen", so the allocator handed out the next free 19xx slot and a 1930/1940/1941 mapping came back as 1942-1946 on every consent renewal, roughly quarterly per connection. Match on the IBAN instead. resolvePsd2LedgerAccount() finds the existing cash_accounts row by normalized IBAN before allocating, and upsertFromPsd2 promotes that row in place rather than inserting a second one, so it keeps its id and its linked transactions and is re-pointed at the connection that just authorized. The previous holder's connection status is deliberately ignored: one IBAN is one physical account, and the old row often still reads 'active' because the bank killed the session without telling us. The allocator also stopped treating a 19xx number as free just because no cash_accounts row holds it. A chart imported from SIE carries the company's real bank accounts by name with no PSD2 row behind them, which is how a SEK company account got proposed as an unrelated brokerage account. Overflow now skips chart-occupied numbers, falling back only when nothing unnamed is left. Dead connections kept rendering as "Aktiv": status only ever changed when a transaction fetch failed, so a session killed bank-side stayed healthy-looking with a stale last_synced_at while the user read old balances as current. Add probeSessionHealth() and run it in the daily cron over every connection that run did not prove alive, including the ones the loop skips silently (capability gate, all accounts deselected) and the ones parked in pending_selection that the cron never looked at. It acts only on a definite dead answer; anything ambiguous leaves the row alone, since a wrong flip costs a full BankID re-authorization. The all-accounts-deselected branch is reclassified 'synced' to 'skipped' for the same reason: it never contacts the bank, so it must not count as proof of life. The settings row warns when an active connection has not synced in three days or has never synced. Which company a connection belongs to was invisible. Everything was already scoped to ctx.companyId, so there was no cross-tenant leak, but a bank authorized while the wrong company was active looked identical to the right one. Name the company on the connect surface and in the account picker, and say where the connection went when the callback lands under a different active company. Warn (bypassably) before authorizing a bank where the same user already holds live connections in other companies: several ASPSPs allow one active AIS session per login, so the new authorization can kill the others. The history start date already defaulted to the fiscal-year start; the card above it recommended a mid-year date and contradicted the selected option. It now states the fact and offers the shortcut without presenting it as advice. Not addressed: sharing one PSD2 session across companies. company_id is the tenancy anchor on bank_connections and cash_accounts hangs off (company_id, bank_connection_id), so that needs the session to become its own entity. Not advisory: both routes reject it outright with MATCH_INVOICE_ALREADY_PAID / MATCH_SI_ALREADY_PAID, so no override could succeed. - The supplier detail card now shows remaining_amount like the customer branch, instead of total. Three properties are load-bearing and covered by tests. The kick is never awaited, because eventBus.emit is awaited at ~99 call sites including journal_entry.committed and each delivery can burn a 10 s receiver timeout. It coalesces per function instance, so a bulk booking that emits once per row does not schedule one claim round trip per row. It claims 5 rows rather than the cron's 50, because it runs on the tail of a user-facing request. Double delivery is not a risk: claim_due_webhook_deliveries already claims FOR UPDATE SKIP LOCKED and flips rows to in_flight in the same statement, so a kick racing the cron sees disjoint rows. None of the three mask checks recognised it, each having its own '-1234'-only copy, so such a customer could not be edited in ANY field: name and address edits 400'd on a personnummer the user had no way to correct. All three now share one pattern from the new crypto-free lib/customers/mask-personal-number.ts, which the client form can import. Typing a fresh personnummer overwrites the unreadable value, which is the only repair possible: the rejected writes failed whole INSERTs, so there is nothing to backfill. The value was write-only by construction. GET /api/customers/{id}/personal-number is the deliberate drill-in, mirroring the employee convention, gated on the write role because .compliance/ropa.yaml listed no_full_value_read_endpoint as a safeguard for this column; that entry is rewritten rather than left stale, and reveals log actor and customer id but never the value. The comment claiming SALES_OUTPUT_VAT_SHORTFALL reads base25/12/6 was wrong and is corrected. That check derives its expected base from the output-VAT rutor (ruta10/0.25 + ruta11/0.12 + ruta12/0.06); nothing reads the per-rate bases, which are reporting metadata. So the incomplete split never affected a filed return or a warning, only the breakdown. style: remove em/en dashes repo-wide, add CLAUDE.md rule against them (#890)
Em dashes (—) and en dashes (–) had spread across comments, docs, tests, and a few UI strings, reading as AI-generated boilerplate rather than house style. Replaced each with punctuation matching its context: colon for explanatory clauses, comma for asides, plain hyphen for numeric/legal ranges (e.g. "21-23§"), "to"/"till" for date ranges, parentheses for paired-dash asides. messages/en.json and messages/sv.json were fixed by hand together to keep sv/en in sync. Left untouched where the dash is the functional subject rather than decorative punctuation: date-range-parser.ts's separator regex, charset-repair.ts's CP1252 byte-mapping table (and its test), the SIE encoding mojibake docs, generic-csv.ts's minus-sign normalizer, the agent system-prompt files that already instruct against em dashes, and a golden iXBRL test fixture compared byte-for-byte. Also fixes two bugs surfaced along the way: an off-by-one in ApiKeysPanel's scope-label split (a leftover from an earlier partial pass), and a charset-repair test that had lost the literal en-dash it exists to verify. Regenerated the agent atom seed migration (skills:generate) since 27 SKILL.md files changed. Replace PSD2 bank integration as the default with file-based bank import (CSV/XML), which better suits Swedish sole traders and small companies.
