fix(security): audit remediation 2026-09-01: api_keys identity, viewer gates, OAuth binding, XSS, MFA gate (#2155)
* fix(security): bind api_keys to the caller, lock hash-as-bearer RPCs and provider token tables Security audit 2026-09-01, critical items. - api_keys INSERT requires user_id = auth.uid() again (an admin could forge a key for any co-member and act as them in every company they belong to); SELECT is own-keys-or-admin; a BEFORE trigger freezes the identity and credential columns against user-session UPDATEs. - rotate_mcp_refresh_token and validate_and_increment_api_key become service_role only: they match rows by a presented SHA-256, so a hash readable by co-members was a bearer credential. - validate_and_increment_api_key fails closed when the key's user is no longer a member of the key's company. - provider_consent_tokens and provider_otc: the DELETE policies collapsed to "caller has any team row" (correlated subquery on a non-existent team_members.company_id). All member policies dropped; service_role only, matching every existing code path. fix(white-label): accept byrå-team invites before landing, so admins reach /clients (#2002)
A newly-invited byrå admin/member who signed up with email+password landed on /onboarding instead of the cockpit. Root cause: team-invite acceptance lived only in POST /api/team/accept, which the email-confirmation signup flow never reaches before the dashboard (no session for the register page's client-side accept), while the auth callback and the onboarding/select-company recovery only understood company_invitations. So the invitee's byrå membership did not exist when landing resolved, and they were funneled into creating a company. - New shared helper acceptPendingTeamInviteByToken (lib/company/pending-invites) is the single server-side implementation of team-invite acceptance. - POST /api/team/accept delegates to it; HTTP contract unchanged. - /auth/callback accepts a team invite BEFORE the silent-team check and before resolveLandingDestination runs, so an owner/admin resolves to /clients; the invite cookie is cleared on success, kept otherwise for the retry. - acceptPendingInviteByToken (onboarding/select-company recovery) tries the company path, then falls back to the team helper. - hasPendingInviteForEmail checks both invite tables, so a tokenless byrå invitee is not misread as a first-timer. fix(mfa): hard-navigate after TOTP enrolment so the user is not stranded on the QR screen (#1948) (#1984)
leave() in /mfa/enroll ended with router.push(returnTo) followed by router.refresh(). Enrolling raises the session to aal2, which lib/supabase/middleware.ts only re-evaluates on a fresh document request; the push and the refresh raced, the refresh won, and the user was left on the QR screen with 2FA already active and no way forward but the address bar. Always window.location.assign(returnTo) instead, the way the same function already did for /api/ destinations and /mfa/verify does for its invite and route-handler paths. returnTo is already validated by safeReturnTo, so the unconditional hard navigation stays same-origin. A brand domain belongs to the partner's people (founder decision
2026-08-27): only allowlisted or invited users may create an account on
an invite-only brand domain; everyone else is shown an interstitial that
sends them to the canonical Accounted signup.
- brands.signup_mode ('open' default / 'invite_only') +
brand_signup_allowlist (lowercase emails, team-scoped RLS, owner/admin
writes) + create_company_for_brand_signup RPC, with pg-real coverage
- server-side gate (lib/auth/brand-signup-gate.ts) enforced on every
signup path: email signup moved to POST /api/auth/signup (the browser
used to call GoTrue directly, so a client-side check would be
bypassable), BankID gated in /bankid/complete, Google covered by the
dashboard layout's brand-domain bounce
- company invites bypass the allowlist: the invite is the authorization
- register page interstitial on gated brands (no email in the outbound
URL), sv+en strings
- dashboard layout bounces non-belonging sessions off gated brand hosts
to the canonical domain (navigation rule like WL-01, not a security
boundary)
- allowlisted signups' onboarding-created companies attach to the
brand's byra team via the new RPC, so WL-01 homes them on the brand
domain; the allowlist entry recorded by an owner/admin stands in for
the WL-15 admin gate
- byra cockpit page /clients/access + /api/clients/signup-access to
manage the mode and the allowlist
All existing brands default to 'open': behavior is byte-identical until
a brand is flipped to invite_only.

* fix(white-label): rollback brand-signup company with the service client
Skeptic (correctness) found that a brand-signup company created under the
service role rolled back with the cookie-session client: `companies` has
RLS and no FOR DELETE policy, so the delete was a silent 0-row no-op,
stranding a member-less ghost company on the partner's byra team. Pass an
optional rollbackClient to createCompanyCore and hand it the service
client on that path; user_preferences.active_company_id then clears itself
via its ON DELETE SET NULL FK once the company row is actually deleted.
Also map a validateBody 400 (flat envelope, no code) on the register page
to the specific email-invalid field message instead of the generic one,
since the client already pre-gates password strength.

- Fail safe on a brands-table error (CodeRabbit CWE-285): the gate treated a
failed resolveBrandByHost as an unbranded host, opening invite-only signup
during a transient DB blip. resolveBrandResultByHost now distinguishes
"no brand" from "lookup failed"; the gate returns lookupFailed and the
email + BankID routes answer 503 (retry), never creating an account.
- pg-real: the RLS delete test seeded its row inside withUserContext, which
always rolls back, so the owner DELETE saw zero rows. Seed on the superuser
pool instead.
- Anonymize every test/fixture brand to the repo's existing synthetic
placeholder (Siffra / app.siffra.se): no real partner names in code.
- SignupAccessManager: functional setData updates so a concurrent mode
toggle and an add/remove do not clobber each other's snapshot (CodeRabbit).
- Route a transient-error message through i18n instead of the raw envelope
(raw-user-error guard); new register.error_temporary sv+en.

Rename the placeholder brand in the four new brand-signup test files to a
clearly-fake, partner-unrelated name (Testbrand / app.testbrand.example);
the previous placeholder echoed a real partner. Scoped to files this PR
creates; the repo-wide legacy placeholder is left for a separate cleanup.
Also record in DECISIONS.md that the feature ships accepting the
low-severity allowlist-enumeration residual (captcha-free 403 vs 200 on
the signup endpoint), with rate-limiting as the follow-up option.

Two chunks rode along in the first-load JS of almost every dashboard route:
the full BAS 2026 chart (315 KB uncompressed, in 81 route manifests) and
the browser polyfill for Node's crypto/vm/Buffer (327 KB, in 26 routes
incl. login and register). Neither was needed on first paint; both got
there through static imports of helpers that happen to live next to code
that needs the data or the builtin.
Node polyfill (4 pure splits, behaviour unchanged, re-exported from the
original modules for server callers):
- lib/auth/bankid-flags.ts: isBankIdEnabled (login, register, security
settings imported it from bankid.ts, which imports crypto).
- lib/import/bank-file/formats.ts: the format registry + detection (the
import history imported getFormat from parser.ts, which hashes).
- lib/salary/personnummer-format.ts: parsing/validation/formatting (the
employee forms reached the encrypting personnummer.ts via tax-column).
- lib/auth/api-key-scopes.ts: scope catalogue, groups, tool map, helpers
(the API key panel imported STAGING_SCOPES from the key generator).
BAS chart:
- lib/bookkeeping/bas-lazy.ts + use-bas-reference.ts: the chart becomes a
dynamic import, fetched once per session after first paint; components
that show BAS names/descriptions call useBasReference() and re-render
when it lands. Until then (and on the server) only the hardcoded
account-descriptions answer, so SSR and hydration agree.
- lib/bookkeeping/bas-labels.ts: class/group labels out of bas-reference.ts
(account-descriptions needed a label and paid for the whole chart).
- lib/bookkeeping/bas-account-numbers.ts (generated, ~11 KB) +
scripts/generate-bas-account-numbers.ts (--check) + parity test:
isStandardBASAccountNumber for AddAccountDialog/ChartOfAccountsManager.
- lib/bookkeeping/account-classifier-{heuristic,client}.ts: the BAS-aligned
heuristic shared by the server classifier and a client variant that uses
the lazy chart.
- lib/bookkeeping/invoice-accounts.ts: INVOICE_FX_RATE_MISSING,
InvoiceFxRateMissingError, getRevenueAccount, getOutputVatAccount out of
invoice-entries.ts, whose engine import pulled account-backfill and the
chart into SendInvoiceDialog/PaymentBookingDialog.
- CorrectOpeningBalanceDialog re-seeds names when the chart lands;
OpeningBalanceRowEditor builds its Fuse indexes lazily; the
ChartOfAccountsManager BAS-katalog tab awaits the chunk.
Tooling:
- scripts/perf/client-import-closure.mjs: static import closure of every
'use client' module with the shortest chain to a target (file or bare
specifier); found every path above without a build.
- scripts/checks/client-node-builtin.mjs wired into check:guards: a client
module reaching a Node builtin is a hard failure (0 today).
Left as is: invoices/[id], its credit page and SendInvoiceDialog still
reach the chart through lib/invoices/issue-credit-note -> invoice-entries
-> engine -> account-backfill; splitting the engine is out of scope here.

* fix(auth): move the BankID flow into a signed, single-use, confirm-on-resume cookie
A user's BankID signup identified successfully four times and created no
account. His screenshots show four tabs, one on the finished "Verifierad med
BankID, ange e-post" step, and the tab he was looking at showing the idle
button. Prod agreed: no bankid_identities row, no auth.users row.
On iOS outside plain Safari the BankID return URL is handed to the OS, which
opens a NEW tab. The session lived in per-tab sessionStorage, so that tab
started empty and rendered the start button while the completed flow sat
stranded. Login hid it (self-finishing, cookie-backed session); signup waits
for a human to type an e-mail into the stranded tab, so it dies there.
The session id is no longer handed to the browser. It lives in a signed
__Host- HttpOnly cookie set at /start; /poll, /complete, /link and /cancel
read it. Cookies are shared by every tab of the origin, which is what the
handoff needed. The id had to leave the client because it is an
unauthenticated bearer credential: /poll was skipAuth and returned
user.personalNumber, and /complete with mode 'login' returns a tokenHash that
verifyOtp turns into a session, MFA skipped for bankid_linked accounts.
A completed identification must never be consumed by whoever merely opens the
page. A shared cookie plus a shared machine means the tab that finds a
completed flow cannot prove the person at it is the one who made it, and no
client-side token can prove otherwise: nothing survives an iOS same-tab reload
yet dies on reopen-closed-tab / session restore / tab duplication. So a resume
is never automatic. The mount probe routes any found live flow to a confirm
card ("Fortsätt bara om det var du") that reveals no name, and only that click
polls and consumes. Auto-consume happens only inside the live component
instance that called startSession (desktop QR; the pre-navigation mobile
launch), which by construction is the originator. Cost: one tap after
returning from the BankID app on iOS, exactly where the reported bug lives;
desktop and Android never hit the resume path.
The rest is defence the four review rounds proved load-bearing:
- __Host- with Path=/ and unconditional Secure, so a script cannot plant the
same name at a longer path; readBankIdFlow fails closed on duplicates and on
a malformed percent-escape.
- Single-use is a unique index (bankid_consumed_sessions), claimed before
generateLink, not a Set-Cookie. Fail-closed on any non-23505 error, so the
migration MUST be applied before the code.
- A link flow requires auth at /start and pins userId; /link rejects a flow
owned by anyone else, before any TIC call. mode is pinned and /poll rejects a
body mode that does not match, so a login session cannot finish through the
signup panel. /poll withholds the holder name from a probe. The 900s
verified-step window is capped by MAX_TOTAL_LIFE from a signed startedAt.
/poll never clears the cookie (an untargeted Set-Cookie would delete a newer
flow); only /cancel and terminal /complete + /link exits clear. Avbryt holds
a 'cancelling' state until /cancel resolves so a new /start cannot race the
clear. Session id is logged only as an 8-char prefix.
The launch is untouched: iOS keeps its return URL, Android keeps redirect=null
(#194 closed that path deliberately).

Error display drops the boxed banner everywhere: credential failures render as one destructive sentence directly under the password field (fields keep aria-invalid), and the reset-password action surfaces from the second consecutive failure. BankID/Google failures, callback errors and the session-timeout notice are single quiet lines at the top of the panel (AttnLine for the informational one). Also: password visibility toggle, webkit autofill repaint to the theme surface, auth pages move from the gradient background to the app frame tone, register/MFA/reset get the same backdrop for cross-page coherence, and Skapa konto moves out of the panel into a footer line. * fix(auth): provision invitees server-side when signups are disabled Self-hosted installations with GoTrue disable_signup broke the invite flow silently: invitees without an account were routed to /register, where supabase.auth.signUp fails with "Signups not allowed for this instance", surfaced only as a generic toast. New server-only env flag AUTH_SIGNUPS_DISABLED (documented in .env.example) mirrors the GoTrue setting. When true, POST /api/company/members/invite checks check_email_exists and, for invitees without an account, provisions one via auth.admin.inviteUserByEmail with a redirect back to /invite/<token>, before the Resend email and before the invitation row is written so a provisioning failure leaves nothing half-created and the admin can retry. The response now carries user_provisioned alongside email_sent, and a provisioning failure returns 502 with a Swedish message mapped through getErrorMessage instead of a silently-successful invite. /auth/callback now routes type=invite verifications to /reset-password (the existing set-password surface) instead of dropping the passwordless user on the dashboard, and preserves the invite token from next=/invite/<token> as the pre-auth invite cookie so the existing reset-password invite handoff accepts the membership right after the password is saved. getErrorMessage learns two GoTrue patterns: "Signups not allowed" (account creation closed on this installation, contact your inviter or administrator) so the /register dead end is explained even for flows that bypass provisioning, and "Error sending ... email" (GoTrue SMTP not configured) so the 502 above is actionable. Hosted is untouched: the flag is unset there and every new code path is gated on it. Any caller holding a company_id and an import id could hard delete another tenant's verifikationer. Confirmed live in production. Applies the same fail closed owner/admin guard that undo_sie_import already carries (migration 20260624120000), resolving the actor from COALESCE(p_user_id, auth.uid()) so it denies when the role is NULL, then revokes EXECUTE from PUBLIC and anon. search_path and the raised statement_timeout are restated, since CREATE OR REPLACE drops settings that are not repeated. userId is a required parameter on replaceSIEImport: the service client has a NULL auth.uid(), so a caller without an explicit actor now fails to compile rather than hitting the closed gate at runtime. Also scopes getConsent to an owning company, closing a cross tenant status oracle where the preview and migrate paths echoed a consent's status before the scoped check ran. Phase C, which drops the old policies, is gated on the backfill reporting zero remaining legacy prefix objects. The policy compares the company segment as text rather than casting to uuid the way sie-files does: this bucket holds keys whose second segment is not a uuid (MCP audit packages), and Postgres does not guarantee the bucket prefix qual runs before the cast, so a planner reordering would raise 22P02 and fail the whole query instead of filtering the row out. deleteDocument now removes both candidate keys. Removing only the stored pointer would leave a readable orphan copy of a document the user asked to erase. The backfill script is included but has never been run. It defaults to dry run, refuses .env.local by name, and verifies each copy is readable and SHA-256 identical before repointing the row. PostgREST compiles an embed with filters on the embedded side into a correlated INNER JOIN LATERAL with a parameterized LIMIT, which stops Postgres reordering the join, so each query walked the whole journal_entry_lines table across all tenants. Driving from the entries side instead turns that into two indexed round trips. Converted sites keep their existing shape: the helper reattaches the parent entry under the same key the embed produced. Several conversions also remove a latent silent truncation where an unpaginated query was capped at PostgREST's 1000 row ceiling. Two deliberate exceptions. The free text ilike legs of the MCP display query stay on the embed, because each is capped at legLimit and that cap drives the truncation contract the tool reports, while the helper is unbounded. The accounts route moves to the existing get_account_usage_counts RPC instead, since its embed was a head count and the helper returns rows. commitEntry's write path is untouched: the change there is confined to the read query of the pre-commit dimension rule check. Every business date is still on the row, and the invoices list gains date_from and date_to filters so a date range is still reachable; the other two already had them. The tests use an in-memory PostgREST that actually evaluates the filters, because the repo's pass-through mock cannot catch this class of bug: the bug is that the filter is never sent. They walk to exhaustion with a hard iteration cap, so an unterminated walk fails instead of hanging. Installing into the parent of the checkout resolves only that one package, so an unrelated peer conflict can never take the gate down again. Node still finds it because ESM bare specifiers walk up parent node_modules; NODE_PATH would not have worked, as it is CommonJS only. --legacy-peer-deps was rejected because it masks future genuine peer conflicts and still reifies the full tree. The same step's SDK version is aligned from 0.31.0 back to the 0.29.1 that package.json and check:guards enforce after the streaming outage. That drift went unnoticed because the pin guard only inspects package.json and the lockfile, never workflow files. Two such entries were already present in the crontab for extensions absent from the preset, which settles the intent. documents/verify is treated as drift rather than a self hosted concession: the weekly cadence was present in the hosted crontab too, and the run is capped at 200 documents walking a nulls-first queue, so weekly drains the integrity queue seven times slower on a check that exists for BFL retention. webhooks/dispatch keeps its per minute cadence, adding 1,440 requests a day on self hosted. A gentler tick would silently stretch the first retry, since the retry ladder opens at 60 seconds. SCHEDULE_OVERRIDES is the one line place to change that. A parity test asserts the path sets match minus a documented exclusion list, and ratchets three cron routes that are currently scheduled nowhere so they are named rather than silently rotting. Redaction moved out of lib/logger.ts into a leaf module that both the logger and the sink import, so there is one denylist and no path from application data to a third party can skip the personnummer regex, including direct sink calls that bypass the logger. That matters here because these logs carry personnummer and financial data. verifyCronSecret now reports its own 401s, which covers all 16 jobs without touching a route file and catches the case where CRON_SECRET is rotated without updating the scheduler and every job silently 401s forever. The threshold is one failure rather than the backup alert's three: suppressing the first occurrence is precisely how an outage stays invisible. The seven misleading comments are corrected to describe what the code actually does, including the two cases that still are not covered: the client side one, since the sink is server side, and a warn level call that is not forwarded. An invited user who registered via BankID was funneled into creating a company instead of joining the one they were invited to: the register page's BankID path never processed the gnubok-invite-token cookie (unlike the login, MFA-verify, and auth-callback paths). Observed in production 2026-07-24. - register: BankID signup now accepts the pending invite before routing to /select-company, mirroring the login page's BankID path. - lib/company/pending-invites: acceptPendingInviteByToken retries a missed acceptance from the cookie (pending + unexpired + email match, same rules as POST /api/team/accept); hasPendingInviteForEmail detects a stranded invitee whose cookie is gone. - /onboarding and /select-company retry acceptance from the cookie and redirect to the dashboard on success, making the auth callback's long-promised fallback real; with no cookie but a pending invitation, both surfaces show a 'join via the link in the invitation email' hint instead of silently asking the invitee to create a company. - No new accept path without the token: the hint deliberately points back to the mailed link, so mailbox possession stays required and no company name is leaked to unverified emails. Corporate mail scanners (Microsoft Defender SafeLinks) follow links in auth emails and burn the one-shot recovery token before the user sees the mail (#1099, first hit: Deepgrid 2026-07-21, verify from an Azure IP 23s after send). /reset-password now has three entry modes: - set-password: recovery session exists (legacy /auth/callback links keep working unchanged) - confirm-link: the email link carries ?token_hash= and verification runs ONLY on an explicit button click; scanners render pages but do not click buttons, so the token survives scanning - enter-code: email + 6-digit {{ .Token }} code typed manually, the fallback when no link works at all The Supabase recovery email template switches to {{ .SiteURL }}/reset-password?token_hash={{ .TokenHash }} + {{ .Token }} AFTER this deploys (template content in the PR); the link then never touches gotrue's GET /verify endpoint, leaving nothing to detonate. A failed email-verification link redirected to /login?error=auth_error with no flow context, so the login page framed every callback failure as a broken password-reset link and pushed new users into a reset form for an account that was never confirmed. The callback now forwards a coarse flow hint (recovery vs signup); the login page renders confirmation copy without the reset CTA for the signup case. The new copy names the likely cause (link opened in a different browser than signup, or a one-time token consumed by a mail scanner) instead of only "expired or already used". Silent-team creation is also wrapped in try/catch so a transient insert failure cannot turn an otherwise-successful first-time confirmation into a 500. style: remove em/en dashes repo-wide, add CLAUDE.md rule against them (#890)
Em dashes (—) and en dashes (–) had spread across comments, docs, tests, and a few UI strings, reading as AI-generated boilerplate rather than house style. Replaced each with punctuation matching its context: colon for explanatory clauses, comma for asides, plain hyphen for numeric/legal ranges (e.g. "21-23§"), "to"/"till" for date ranges, parentheses for paired-dash asides. messages/en.json and messages/sv.json were fixed by hand together to keep sv/en in sync. Left untouched where the dash is the functional subject rather than decorative punctuation: date-range-parser.ts's separator regex, charset-repair.ts's CP1252 byte-mapping table (and its test), the SIE encoding mojibake docs, generic-csv.ts's minus-sign normalizer, the agent system-prompt files that already instruct against em dashes, and a golden iXBRL test fixture compared byte-for-byte. Also fixes two bugs surfaced along the way: an off-by-one in ApiKeysPanel's scope-label split (a leftover from an earlier partial pass), and a charset-repair test that had lost the literal en-dash it exists to verify. Regenerated the agent atom seed migration (skills:generate) since 27 SKILL.md files changed. Fixes #701. setActiveCompany upserted active_company_id without checking the result, then set the gnubok-company-id cookie unconditionally. A failed write — including an RLS-filtered UPDATE, which affects zero rows without raising an error — looked like a successful switch: switchCompany returned {}, the UI hard-reloaded, and middleware (which reads user_preferences, not the cookie) resolved the old company. - setActiveCompany now verifies the upsert with .select().single() and throws a typed CompanyContextError ('not_member' | 'persist_failed'); the cookie is only set after the write is confirmed, so it can no longer diverge from the database. - switchCompany logs the failure and returns distinct error codes instead of reporting every failure as a permissions problem. - CompanySwitcher now shows a destructive toast on failure (it previously failed with no feedback); BankIdCompanyPicker translates the codes. Messages added to sv/en under company_switcher and select_company. - The remaining fire-and-forget user_preferences writers (middleware fallback write-back, team invite accept, auth callback invite accept) now check and log errors; non-fatal by design since each has a working fallback path. - New tests cover every failure mode, including cookie-not-set on a failed write and the silent zero-row write caught by the read-back. Bigger PR per the user's request. Lands the remaining two journal-entry-
centric action verbs together — they share the same lifecycle pattern
established in :mark-sent (idempotent, dry-runnable, scope-gated,
warnings on partial-state failures).
POST /api/v1/companies/:companyId/invoices/:id/mark-paid
- Books a payment against a sent / overdue invoice. Updates status to
paid (or partially_paid when remaining_amount > 0). Three booking paths:
- Faktureringsmetoden (accrual default): Debit 1930 / Credit 1510 via
createInvoicePaymentJournalEntry — settles AR.
- Kontantmetoden (cash): Debit 1930 / Credit revenue + Credit VAT via
createInvoiceCashEntry — revenue recognition happens HERE under cash.
- Custom lines (partial payment): caller-supplied balanced journal lines
via createJournalEntry directly. Validated for balance (sum debits ==
sum credits, both > 0) → 400 INVOICE_PAID_LINES_UNBALANCED otherwise.
- Optional body: { payment_date?, exchange_rate_difference?, lines? }
- Race-condition guard: status update matches .in(['sent','overdue',
'partially_paid']) so a concurrent payment returns 409 INVOICE_PAID_RACE.
- Emits invoice.paid (new event type, added to lib/events/types.ts with
paymentAmount + paymentDate in the payload).
POST /api/v1/companies/:companyId/invoices/:id/credit
- Issues a kreditfaktura against a sent / paid / overdue invoice
(ML 17 kap 22–23§). Creates a NEW invoice row with:
- invoice_number = "KR-<original>"
- credited_invoice_id = original id
- status = 'sent'
- All amounts negated (subtotal, vat_amount, total, items quantities/totals)
- Items mirror the original with negated values; inserted in a separate
step with company-scoped rollback DELETE on failure.
- Flips original invoice to status='credited'. Warns ORIGINAL_NOT_FLIPPED
if the flip fails (the credit note still exists; operator reconciles).
- Posts reverse journal entry via createCreditNoteJournalEntry (accrual
only; cash basis defers to refund time).
- Emits credit_note.created (existing event in the bus).
Both endpoints:
- Use the established wrapper + Idempotency-Key + dry-run + warnings
pattern from :mark-sent.
- Validate document_type (no delivery_notes), credited_invoice_id (no
recursive credits), and status before any mutation.
- Use explicit column projections (no SELECT *).
- Sanitize pg_message from client responses (kept in logs).
- Emit error-level logs on partial-state failures + surface warnings to
the caller via meta.warnings.
Event types union (lib/events/types.ts) gains invoice.paid; credit
uses the existing credit_note.created event.
URL convention: plain /verb subpaths (e.g. /invoices/:id/mark-paid),
consistent with :mark-sent. Stripe/QuickBooks pattern, not the
AIP-style :verb that Next.js routing fights.
17 new tests covering happy paths (accrual + cash for mark-paid),
custom-lines balance validation, dry-run preview, document-shape
guards, scope, idempotency, race conditions, and credit-of-credit /
delivery-note rejection.
3194/3194 vitest pass; build clean; lint clean on v1 paths.

PR #455 review fixes:
- Greptile P1 (CLAUDE.md architecture rule): API routes that emit events
via eventBus must call ensureInitialized() at module level to wire
extension event handlers. Neither :mark-paid (invoice.paid) nor :credit
(credit_note.created) had it — nor did the already-merged :mark-sent,
POST /invoices, POST /customers, etc. Fixed once at the wrapper layer:
ensureInitialized() now runs at module import of lib/api/v1/with-api-v1.ts,
so EVERY v1 route gets the init at import time. Single source of truth
prevents future routes from forgetting (idempotent guard makes the
repeated call safe). Cleaner than per-route copy of the call.
- Swarm PI1.3 (low): 0.005 epsilon in mark-paid was undocumented. Added
a comment explaining: after rounding to 2 decimals, newRemaining is in
steps of 0.01; values ≤ half-an-öre only arise from float artefacts.
Pushing back (recurring triage, consistent with prior PRs):
- V8.2.1 + CC6.3 × 4 "ctx.companyId vs params.companyId mismatch" —
impossible by construction. The wrapper sets ctx.companyId FROM the URL
params after the membership check. They are guaranteed equal.
- V2.3 + A.8.15 + A.8.28 atomicity / floating-point / partial-failure
alerts — same architectural / cross-surface deferred work as prior PRs;
matches internal /api/invoices pattern precisely.
- V4.5 account_number allowlist — engine validates it.
- V2.4 idempotency TOCTOU — wrapper handles via DB unique constraint.
- Art.5(1)(f) PII in logs, A.8.11 dry-run preview scope, A.8.15 partial-
failure naming, test scope coverage — all recurring triage.
Password-recovery flow fixes (included per request — pre-existing
working-tree changes the user authored):
- app/(auth)/auth/callback/route.ts: when the callback exchanges a
recovery token (type='recovery' or next='/reset-password'), redirect
directly to /reset-password instead of running onboarding/MFA/
dashboard checks. Previously users clicking the password-reset email
got bounced through onboarding.
- lib/supabase/middleware.ts: /reset-password no longer bounces
authenticated users to / (the recovery flow lands here with a fresh
session by design — the user is *supposed* to call updateUser({
password }) on this page).
- app/(auth)/login/page.tsx: shows an error banner when ?error=auth_error
is set (expired/used recovery link), with a button to request a new
one. Wrapped the page in <Suspense> because useSearchParams() now
forces dynamic rendering (Next.js 16 static-prerender bail-out
otherwise).
- app/(auth)/auth/callback/__tests__/route.test.ts: new test file
covering the recovery callback path.
3197/3197 vitest pass (3194 prior + 3 from the new auth-callback tests).

Real correctness fix from Swedish-compliance review on PR #455. When no
customLines is supplied, mark-paid previously defaulted paymentAmount to
typed.total. Combined with the race-condition guard that allows the
status UPDATE to flip a partially_paid invoice to paid, this could
over-credit AR in a race scenario:
1. Invoice in 'sent' status, total=12500, remaining=12500.
2. Concurrent partial payment lands first → status='partially_paid',
remaining=7500.
3. The full-payment request's pre-flight saw 'sent' and passed; its
UPDATE matches partially_paid (race guard allows it). With the old
logic the journal entry was for total=12500 against an AR balance
of only 7500 — a 5000 over-credit.
Using remaining_amount as the default eliminates this. Same end state
in the common case (no prior partial); correct booking in the race.

Because BankID proves identity but not email ownership, an attacker who knew a victim's email could bind their own BankID to the victim's account and then log in via BankID (which skips TOTP MFA). Now signup returns 409 account_exists; the register page shows a Swedish error toast and redirects to /login so the user can authenticate with their password first and link BankID from settings (via the authenticated /bankid/link route that already exists). Covered by new bankid-complete.test.ts with an explicit regression test asserting no side-effects occur on the account_exists path.
